Consent Form

By proceeding with booking or receiving services at The Beauty Rezort Medi Spa, you acknowledge and agree to the following:

 

 

1. General Consent

 

I consent to receive services provided by The Beauty Rezort Medi Spa, including but not limited to: facials, chemical peels, skin rejuvenation treatments, body contouring, massage therapy, lash and brow services, waxing/sugaring, and other aesthetic and wellness treatments, whether currently offered or added in the future.

 

 

2. Understanding of Treatments and Results

 

• I understand that all treatments are elective and aesthetic in nature and are not a substitute for medical care, diagnosis, or treatment.

• I acknowledge that no guarantees have been made regarding the outcome of any service, and multiple sessions may be necessary to achieve optimal results.

• I understand that individual outcomes vary depending on factors such as skin type, body response, lifestyle, and adherence to aftercare instructions.

 

 

3. Health Disclosure

 

• I affirm that I have accurately disclosed all relevant medical history, current health conditions, medications, allergies, previous procedures, and other pertinent information.

• I understand that failure to disclose accurate health information may increase risks and releases The Beauty Rezort Medi Spa and its staff from any associated liability.

 

 

4. Risks, Side Effects and Discomfort

 

• I understand that some treatments may involve risks such as redness, sensitivity, swelling, peeling, bruising, or mild discomfort.

• I agree to immediately inform my technician of any discomfort, adverse reaction, or unusual sensations during or after the procedure.

• I understand that if necessary, treatment will cease in response to any adverse reaction or upon client request.

5. Service-Specific Acknowledgments

 

I understand and acknowledge that:

 

• Massage Therapy: is for relaxation and relief of muscle tension only; it is not a medical treatment. I have disclosed any injuries or conditions to my therapist.

• Facials / Hydra-facial Treatments: may involve product application, exfoliation, and extraction techniques that may cause temporary redness, irritation, or sensitivity. I am not currently using Accutane or other skin-sensitizing medications.

​

• Chemical Peels: may cause peeling, redness, and temporary sensitivity as part of the normal treatment process.

​

• Body Contouring: is a non-invasive procedure designed to assist in body shaping; results may vary and multiple sessions may be required.

​

• Lash Lift and Tinting: involve chemical solutions; I have disclosed any eye sensitivities or conditions and will follow aftercare instructions to preserve results and minimize irritation.

​

• Waxing/Sugaring Hair Removal: may cause temporary redness, sensitivity, or irritation; I confirm I have not used retinoid or exfoliating products within 48 hours prior to waxing services.

6. Aftercare

 

I have been informed of appropriate aftercare instructions relevant to the treatment(s) I am receiving and agree to follow these instructions to achieve optimal results and minimize potential risks.

 

 

7. No Guarantees

 

I understand that while The Beauty Rezort Medi Spa strives for the best possible results, outcomes cannot be guaranteed.

 

 

8. Photography and Documentation

 

By booking a service at The Beauty Rezort Medi Spa, you consent to photographs and/or videos being taken during your visit and used for marketing purposes, including but not limited to social media, website content, and promotional materials. You may opt-out at any time by informing staff prior to your appointment.

 

 

9. Cancellation and Refund Policy

 

I understand that The Beauty Rezort Medi Spa has a cancellation and refund policy, which I agree to adhere to.

 

 

10. Privacy Policy

 

I acknowledge that my personal information is protected under the spa’s Privacy Policy and is used only for service purposes.
